Breast Enlargement
BREAST ENLARGEMENT / AUGMENTATION / “BOOB JOB”

Breast enlargement (also known as breast augmentation, augmentation mammaplasty and in the media as “boob job”) is a procedure undertaken to enhance the appearance of the breasts creating larger, soft, natural breasts. The procedure involves the placement of implants underneath the breast tissue or the under the chest wall muscles (pectoral). This may help balance the size and shape of the breasts in relative proportion to the rest of the lady’s body.
Am I a good candidate for breast augmentation/enlargement?
Your surgeon will discuss with you in an unhurried manner your reasons for considering breast augmentation.
Breast enlargement is for ladies who:
  • Want to enhance the size of their breasts
  • Want to increase cleavage
  • Want to correct the effects following pregnancy and breast feeding
  • Want to correct differences in size between both breasts
  • Want to correct uneven or deformed breasts
  • Want to correct an absence of breast tissue due to a congenital (birth) problem
  • Want to boost self-esteem and self-confidence.
Are silicone implants safe?
Silicon implants have been in use for many years. In the 1990s concerns were raised about the use of such implants. Consequently, many large international studies were undertaken and it has been shown that there is NO association with breast cancer or with connective tissue diseases. What types of implants are available?
Implants vary according to content (silicone or saline), shape (round or teardrop), surface (textured or smooth), projection (low, medium, high profile).

Where will the breast implants be placed?
Your surgeon will make a detailed assessment at the time of your initial consultation as to where your implant should be positioned (i.e which pocket). Generally, there are 3 locations:

Subglandular:
This means "on top of the muscle" but “underneath the breast (gland)”. This is used when there is sufficient breast tissue to cover the silicone implant. On occasion it may be used to produce an enhanced look.

Subpectoral:
This means "under the pectoral muscle". This is used in women who have insufficient breast tissue and therefore the muscle provides soft tissue coverage of the silicone breast implant which makes it much less noticeable in everyday life.

Dual Plane:
The dual plane technique is a combination of subpectoral and varying degress of subglandular pockets. It involves releasing the muscle partially at the appropriate point so that you can have the benefits of having some of the implant above the muscle and some of the implant below it.

What size should I have?
London Cosmetic Surgeons provides a comfortable setting in which you may trial various sizes of implants. These trial implants are provided by Mr Sojitra and placed inside your bra. This allows you to get an idea as to what size is right for you. The surgeon will guide you on implant size with regard to your body proportions but ultimately the choice of size is up to you, the patient!

How long do they last?
Breast implants are not permanent devices and all will need to be replaced at some point in the future. Modern day implants tend to last on average 10-12 years. Some may last more and some less. Some may need to be changed earlier due to capsular contracture or you may wish to change your implant size through personal choice.

Where will the scars be after breast augmentation?
Scars can be in a number of places. The most common placement by Mr Sojitra will be in the crease underneath your breast (via inframammary incision). Alternatively, it can be placed at the border of the nipple-areola and skin (periareolar incision).

Will I be able to breastfeed after breast augmentation?
Breast feeding tends not to be affected by the placement of silicone implants. Also there is no evidence to suggest that the baby would receive large amounts of silicone through the breast milk. There is more silicone present in the environment and the teats of feeding bottles than in the milk of implant mothers.

Will the implants interfere with mammographic screening?
Breast implants may interfere with mammography and the detection of breast cancer. It is therefore important to let Mr Sojitra know if there is a family history. When having a mammogram it is important you inform the radiographer that you have breast implants so that special views can be undertaken in order to optimize the mammographic process. Breast implants do NOT increase the risk of breast cancer.

You (the Patient’s) Journey

Before the operation
You will have seen Mr Sojitra on more than one occasion to fully answer all your questions. It is important to avoid medications, if possible, such as aspirin and non-steroidal anti-inflammatories (NSAIDs) e.g Ibuprofen (Nurofen), Sodium Diclofenac (Voltarol) for 1 week before surgery. These tablets may increase bleeding during and after surgery. It is advisable to stop smoking 2 weeks before and after surgery from both a wound healing and an anaesthetic perspective.

The operation
You will be admitted to hospital on the morning of surgery and Mr Sojitra along with his consultant anaesthetist will come and discuss the surgical plan again with you and make some marks on your skin with pen. The operation will take approximately 1-1.5 hours and you will wake up with dressings in place before being taken to the ward. In general, breast augmentation is performed as a day case procedure. You will need to have a friend or family member to drive you to and from the hospital and to look after you for the first 24-48 hours following surgery. You may need to stay in hospital overnight if your operation is done later in the day, if you feel too tired to go or if drains are used.

Recovery
You will experience varying degrees of bruising, swelling, and some tenderness all of which improve by the day. Breast implants placed under the muscle are associated with a little more discomfort than when placed under the breast tissue only. Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience. You will be up and moving on the day of surgery. It is important not to get the dressings wet after surgery and you will be advised on showering. Depending on your type of work, you may return to work between 7-14 days after your procedure. No heavy lifting or strenuous exercise for 6 weeks. It will take approximately 3 months before the swelling completely subsides and the final size seen. The scar will continue to improve for up to 18 months and you are encouraged to firmly massage the scar after 4 weeks. Your surgeon will see you 1 week after surgery at which point the dressings will be reduced and again at 2 weeks. At 2 weeks the stitches, which are dissolvable, will be trimmed flush with the skin so they cannot be seen. All dressings will be discarded at this stage. Your surgeon will closely follow your recovery to ensure you are able to resume normal exercising and activities. It is suggested you wear a sports support bra for 6 weeks.

Safety and potential complications?

Scars
Scarring on the whole is not problematic. A scar will always exist and will be hidden in the curve underneath the breast or at the junction of the areola and the skin. Certain individuals e.g. afro-carribean races and red haired individuals are at a higher risk of forming excess scar tissue (keloid or hypertrophic scars). Should these scars develop excellent advice and treatment will be provided by Mr Sojitra.

Infection
This is an uncommon event and occurs in perhaps less than 1% cases. If it does occur and the implant gets infected then the implant may have to be removed and remain out for 3 months. Once everything has settled a fresh implant will be replaced into the breast. Antibiotics are given during induction of anaesthetic and a short course of tablets afterwards.

Bleeding
Rarely, small blood vessels that have been cauterised during surgery open up afterward. This results in a collection of blood (haematoma) that needs to be removed. This involves a return to the operating theatre where the breast implant is removed, the blood collection washed out and the implant replaced. There are no permanent side effects due to this occurrence should it occur.

Capsular contracture
When a foreign material (silicone breast implant) is placed inside the body, the body’s natural response is to react against this and to form a layer around the implant known as the “capsule”. This is entirely normal and in the majority of patients this capsule is soft and cannot be felt. In a minority of patients, this capsule is much thicker and can cause discomfort and alter the shape of the breast. These patients may require further surgery to correct this.

Asymmetry
Most ladies have breasts which are slightly asymmetrical. The surgeon will point out any small differences between the two breasts to you before your surgery as often these small differences become more apparent following surgery.

Nipple sensation
Most ladies experience some alteration in the sensation around the nipples following breast augmentation. They may become over or under sensitive. In the majority of patients sensation becomes normal over time.

Rippling/Wrinkling of the implant
Breast implants can become palpable over time if the soft tissues become stretched over the implant with time and if the skin is relatively thin. At the same time rippling can also be seen in certain ladies.

Implant rupture & silicone leakage
Modern day silicone breast implants are made out of a “cohesive” gel which resembles the texture of jelly. If such implants leak the silicone does not leak like a runny fluid (as was the case in the older implants 20 years ago) but rather it stays near its original position. This potentially minimises that spread of silicone around the body.

Deep Vein Thrombosis (DVT)
Any general anaesthetic procedure is associated with a small risk of DVT which can potentially move to the lungs (Pulmonary Embolus). Here at Cosmetic Surgeons London we attempt to minimise this risk by the use of compression stockings before and after surgery, use of pneumatic compression devices during surgery and motivating the patient to mobilise as soon as possible after surgery.

Breast Reduction

Where will the scars be after breast reduction surgery?
There will always be scars after you have had breast reduction surgery and the exact location of them will depend on which surgical technique is used. However, there are methods of minimising these. Always be sceptical if any surgeon talks you about scar-less surgery. One of the techniques used by Mr Sojitra involves a scar that lies at the junction of the areola (the area around the nipple) and skin and then travels vertically down into the crease under the breast (infra-mammary fold). The scar resembles the shape of a lollipop. This technique is known as the “vertical scar” or “minimal scarbreast reduction and can be employed in suitable ladies. Your surgeon will decide whether this technique is suitable for you and depends on a number of factors such as patient age, skin quality and breast size. In circumstances where this is not possible there is an additional scar that joins the “lollipop” scar to lie in the infra-mammary fold. The scar therefore looks like an anchor or “inverted-T”. The lower part of this scar is hidden beneath the natural lower curve of the breast. The scars fade over many months and you will be provided with excellent scar management advice. Will I be able to breast feed after surgery?
The answer often is yes. However, this is never a guarantee and the milk flow may be altered in some form.

You will need to have a friend or family member to drive you to and from the hospital and to look after you for the first 24-48 hours following surgery.

After the operation, over the ensuing weeks tender firm areas may be felt in parts of the breast. These are referred to as fat necrosis and the majority of these smooth out over time.

Breast Asymmetry
Breast asymmetry
is a difference in breast size or shape or both. If you are one of those unfortunate ladies affected by breast asymmetry then Cosmetic Surgeons London can help you. Many ladies have a degree of breast asymmetry which goes largely unnoticed (i.e. does not need corrective specialist plastic surgery). Your breasts start to grow during the pubertal growth spurt under the influence of oestrogen. During this time it is normal that your breasts may differ from one another in size or shape. Breast Uplift

In many ladies, as they grow older, factors such as pregnancy, breast feeding, and gravity take their toll on their breasts. As the skin loses its elasticity, the breasts often lose their shape and pertness and begin to sag. A breast uplift, or mastopexy, is a cosmetic procedure designed to lift and reshape sagging breasts for a more youthful appearance. Mastopexy can reduce the extra skin present and give your breasts a more firm shape and feel. In some ladies, breast implants are inserted at the same procedure. This allows patients the ability to increase breast size in addition to obtaining a shapelier, firmer look. This is called breast augmentation-mastopexy. Who is breast uplift (mastopexy) for?
Breast uplift is for ladies who are happy with the size of their breasts but want an uplift to raise and reshape them. Some ladies are not happy to find that they have lost a significant amount of breast volume over the years due to ageing. In these ladies, Mr Sojitra can insert implants to increase breast size while, at the same time, enhancing the shape and position of the breasts. When should breast reconstruction be done?

Typically breast reconstruction involves many procedures performed in multiple stages.  Usually, your reconstruction can be carried out at the time of your mastectomy (“immediate” breast reconstruction) or it can be carried out several months to years after your mastectomy (“delayed” breast reconstruction) when all treatment such as chemotherapy and radiotherapy have been completed.  In most cases when the lady is due to receive post-operative radiotherapy as part of the management of her breast cancer, the reconstruction is “delayed”.  Radiotherapy can unfavourably affect the aesthetic result of the reconstructed breast.

What options do you have for breast reconstruction?

There are many ways of reconstructing your breast:

  • Breast Implant / Tissue Expander
  • Autologous Tissues + Breast Implant/Tissue Expander
  • Autologous Tissues (using your own tissues only)
In certain situations, a breast implant is the best way to reconstruct your breast.  In other situations, muscle tissue and fat can be transferred from your back and brought around to the front to create a breast (Extended Latissimus Dorsi flap).  In other cases, the latissimus dorsi flap can be used to cover a breast implant or tissue expander.  In appropriately selected ladies, the gold standard operation is that of a free flap reconstruction.  This involves taking tissue from the abdomen (DIEP flap), buttock areas (S-GAP flap) or other areas and transferring this tissue with its blood supply to the chest.  This involves complex microsurgical techniques to join the blood vessels of the transferred tissue to the blood vessels in the chest.  The tissue is then shaped to create a breast.

Nipple reconstruction is often performed as the final procedure in your series of breast reconstruction procedures once all minor adjustments have been carried out.  It can be carried out under local anaesthesia as a day case procedure.

What techniques are used for nipple reconstruction?

There are a variety of techniques described for nipple reconstruction.  More accurately, we tend to refer to reconstructing the nipple-areolar complex (i.e. the nipple itself and the surrounding areola).  Techniques involve using:
  • tissue from your other nipple (if the other one is large enough) OR
  • local skin and fat to make a new nipple
  • local skin and fat and putting some cartilage underneath your nipple to help maintain projection

What techniques are used for areolar reconstruction?

The gold standard technique is that using tattooing.  Areolar tattooing provides the best colour match and has superseded mediocre old techniques using full thickness skin grafts.  Areolar tattooing can be carried out under local anaesthetic or sometimes without.  More than one session of tattooing is often required to get the best colour match.

You (the Patient’s) Journey

Before the operation
You will have seen Mr Sojitra on more than one occasion to fully answer all your questions.  It is important to avoid medications, if possible, such as aspirin and non-steroidal anti-inflammatories (NSAIDs) e.g Ibuprofen (Nurofen), Sodium Diclofenac (Voltarol) for 1 week before surgery.  These tablets may increase bleeding during and after surgery.  It is advisable to stop smoking 2 weeks before and after surgery from both a wound healing and an anaesthetic perspective. 

The operation
You will be admitted to hospital on the morning of surgery and Mr Sojitra along with his consultant anaesthetist will come and discuss the surgical plan again with you and make some marks on your skin with pen.  You will be required to have a general anaesthetic.  Depending on which breast reconstruction procedure has been chosen, the operation can vary from 1 - 9 hours.  All the stitches used will be dissolving in order to maximise comfort for you.  Stitch removal may cause discomfort for some patients.  Cosmetic Surgeons London always puts your comfort first.  You will wake up with dressings in place before being taken to the ward.  Drains may be used depending on the reconstructive procedure carried out.  In general, you will need to stay in hospital at least 1-2 nights and maybe 1 week depending on the procedure undertaken.  You will need to have a friend or family member to drive you to and from the hospital.

Recovery
You will experience some degree of bruising, swelling, and discomfort all of which improve by the day.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  You are encouraged to walk around the ward as soon as you feel up to it - this may be the same day or a few days later depending on the procedure undertaken.  It is important not to get the dressings wet after surgery and you will be advised on showering.  Depending on your type of work and the operation undertaken you may return to work between 7-14 days after your procedure.  No heavy lifting or strenuous exercise for 6 weeks.  It will take approximately 3 months before the swelling completely subsides and the final result seen.  The scar will continue to improve for up to 18 months and you are encouraged to firmly massage the scar after 4 weeks.  Your surgeon will closely follow your recovery to ensure you are able to resume normal exercising and activities.  It is suggested you wear a sports support bra for 6 weeks.

Inverted Nipple correction

There will always be scars after you have had breast reduction surgery and the exact location of them will depend on which surgical technique is used. However, there are methods of minimising these. Always be sceptical if any surgeon talks you about scar-less surgery. One of the techniques used by Mr Sojitra involves a scar that lies at the junction of the areola (the area around the nipple) and skin and then travels vertically down into the crease under the breast (infra-mammary fold). The scar resembles the shape of a lollipop. This technique is known as the “vertical scar” or “minimal scarbreast reduction and can be employed in suitable ladies. Your surgeon will decide whether this technique is suitable for you and depends on a number of factors such as patient age, skin quality and breast size. In circumstances where this is not possible there is an additional scar that joins the “lollipop” scar to lie in the infra-mammary fold. The scar therefore looks like an anchor or “inverted-T”. Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience. In the majority of cases, our patients are extremely happy with their results and go on to lead transformed lives. There are some potential risks which you need to be aware of. All risks tend to increase in those patients who are overweight and smoke in the 2 weeks before and after surgery.

Safety and potential complications?

Scars
Scarring on the whole is not problematic. A scar will be visible but in the majority of patients fade with time. Certain individuals e.g. afro-carribean races and red haired individuals are at a higher risk of forming excess scar tissue (keloid or hypertrophic scars). Should these scars develop excellent advice and treatment will be provided by Mr Sojitra.

Infection
This is an uncommon problem in breast reduction surgery. However, antibiotics are given during the operative procedure.

Nipple Sensation
Most ladies experience a reduction in the sensation around the nipples following breast reduction. Nipples can become over sensitive in some patients. In the majority of patients there will be some form of altered sensation.

Delayed wound healing
Occasionally some of the areas take longer to heal than usual. An area which tends to be affected in this way is the “T-junction” (where the vertical scar meets the horizontal scar in the breast crease) due to a number of reasons: the skin is thinner here, the entire breast weight places tension on this area during the course of healing. Treatment involves skilful dressing changes by the nurse and often this heals by its own accord.

Asymmetry
Rarely are anyone’s breasts identical and often there are degrees of asymmetry. We certainly aim to create maximum symmetry.

Bleeding
Rarely, small blood vessels that have been cauterised during surgery open up afterward. This results in a collection of blood (haematoma) that needs to be removed. This involves a return to the operating theatre where the blood collection is washed out and the breast sutured back. There are no permanent side effects due to this and your ultimate breast shape will not be affected.

Areola and nipple necrosis

Rarely, part of the areola or nipple may lose its blood supply during the process where the nipple areola complex is moved. The risk of this is very low.

Fat Necrosis

After the operation, over the ensuing weeks tender firm areas may be felt in parts of the breast. These are referred to as fat necrosis and the majority of these smooth out over time.

Deep Vein Thrombosis (DVT)
Any general anaesthetic procedure is associated with a small risk of DVT which can potentially move to the lungs (Pulmonary Embolus). What is an endoscopic brow lift?

Numbness and nerve injury
Numbness in the forehead and scalp area is common in the early stages following brow lifting surgery.  This tends to be temporary and sensation starts to return to normal as the weeks go by.  You may experience a sensation of “pins and needles” which is a sign of recovery.  Sensation is usually normal by 3 months.  Damage to the nerves supplying the facial muscle is reported in the literature but is extremely rare. 

Infection
This is an uncommon problem in brow lift surgery.  As with any surgical procedure, if infection does occur it may be treated with antibiotics. 

Hair Loss
Hair loss is uncommon and may occur in and around the scar and temple areas.  No hair is cut during the procedure.  Any hair loss resulting is usually temporary. 

Delayed wound healing
Rarely, a small part of the scar behind the ear may take a little longer to heal.  This is sometimes the case in smokers.

What types of facelift exist?

  • Skin Only Facelift
    This was the traditional facelift and has been performed for many years.  This technique involves lifting the skin from the underlying tissues and tightening it.  This technique does not give a long lasting result and is not used often by Mr Sojitra
  • Deep Plane (SMAS) Facelift

Beneath the skin of the face lies a deeper layer to which the muscles and soft tissue is attached (Superficial Musculo-Aponeurotic System - SMAS layer).  This layer can be used to elevate the soft tissues of the face to provide a longer lasting result than with the skin only facelift.  This can be achieved by a number of methods.  A portion of SMAS can be removed and tightened (SMASectomy Facelift) or just tightened only (SMAS plication) or by elevating a portion of the SMAS itself (the extended SMAS Facelift).  All these techniques of facelift help to correct jowling and skin laxity around the neck as well as redistributing tissue volume to the cheekbone area.  

  • Subperiosteal Facelift (Mask Lift)

This can sometimes improve facial appearance when the loss of volume is primarily in the midface and cheek regions.  It involves incisions in the hairline and the fat is repositioned over the cheekbones. 

  • The Short Scar Facelift or 'MACS' Lift (Minimal Access Cranial Suspension)

This technique involves the standard scar in front of the ear while the rest of the scar behind the ear is omitted.  It is suitable for patients with mild jowls.  However, this technique is limited in that it cannot correct loose skin around the neck area.  Anybody with a moderate amount of loose skin in the neck will be better served with a slightly longer scar to accommodate this.  However, if tightening is required mainly in the face it provides a useful alternative.  The recovery is quick but the result may not be as long lasting as the SMAS type facelifts. 

  • The Volumetric Facelift
    During the ageing process there is a loss of fat in the face and a hollow appearance.  This technique aims to restores a youthful appearance by the addition of volume (by injecting your own fat) into your face.

Before the operation
You will have seen Mr Sojitra on more than one occasion to fully answer all your questions.  It is important to avoid medications, if possible, such as aspirin and non-steroidal anti-inflammatories (NSAIDs) e.g Ibuprofen (Nurofen), Sodium Diclofenac (Voltarol) for 1 week before surgery.  These tablets may increase bleeding during and after surgery.  It is advisable to stop smoking 2 weeks before and after surgery from both a wound healing and an anaesthetic perspective. 

The operation
You will be admitted to hospital on the morning of surgery and Mr Sojitra along with his consultant anaesthetist will come and discuss the surgical plan again with you and make some marks on your skin with pen.  You will be required to have a general anaesthetic.  The operation will take approximately 3 - 4 hours depending on whether any adjunctive procedures are carried out.  Your surgeon will place pen marks on your face and while you are asleep inject local anaesthetic and adrenaline.  This helps reduce bleeding during your operation as well as reducing pain and improving your post operative comfort.  Cosmetic Surgeons London always puts your comfort first.  The incision is made and your skin is lifted off the cheek.  The SMAS layer and underlying facial muscles are tightened.  The excess skin and fat you have are removed.  Your skin is closed with a combination of fine dissolvable and non-dissolvable sutures.  You will wake up with a light head bandage with or without eyepads depending on whether you have chosen to have adjunctive eye rejuvenating procedure (blepharoplasty)  Some patients require an endoscopic browlift at the same time. Having had a restful night in hospital, the bandages will be removed the following morning.  It is best you take it easy and please note that it is normal for your face to be bruised and swollen.  You will need to have a friend or family member to drive you to and from the hospital and to look after you for the first 24-48 hours following surgery. 

Recovery
You will experience some degree of bruising, swelling, and discomfort around your eyes and cheek all of which improve by the day.  The swelling will tend to increase over the first 48 hours and then gradually subside over the next 2 weeks.  You may put make-up on after a few days if you wish.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  You are encouraged to rest the first few nights.  It is important not to get the dressings wet after surgery and you will be advised on showering.  Depending on your type of work, you may return to work between 7-14 days after your procedure.  No heavy lifting, straining or strenuous exercise for 6 weeks.  It can take a few months before the swelling completely subsides.  The fine stitches will be removed in 5-7 days.  You will be seen 7-14 days after surgery.  Your surgeon will closely follow your recovery. 

Safety and potential complications?

Scars
Scarring on the whole is not problematic.  A scar will always exist and will be in front of the ear an hidden behind the ear.  Certain individuals e.g. afro-carribean races and red haired individuals are at a higher risk of forming excess scar tissue (keloid or hypertrophic scars).  Should these scars develop excellent advice and treatment will be provided by your surgeon. 

Bleeding
Rarely, small blood vessels that have been cauterised during surgery open up afterward.  It is commoner in males and in patients who take blood thinning medications such as aspirin and non-steroidal anti-inflammatories (NSAIDs) e.g Ibuprofen (Nurofen), Sodium Diclofenac (Voltarol).  This results in a collection of blood (haematoma) that needs to be removed.  This involves a return to the operating theatre where the blood collection is washed out and the skin sutured back.  There are no permanent side effects due to this and your ultimate facelift result will not be affected. 

Numbness and nerve injury
Numbness in the face and ear area is common in the early stages following facelift surgery.  This tends to be temporary and sensation starts to return to normal as the weeks go by.  You may experience a sensation of “pins and needles” which is a sign of recovery.  Sensation is usually normal by 3 months.  Damage to the nerves supplying the facial muscle is reported in the literature but is extremely rare. 

Infection
This is an uncommon problem in facelift surgery.  As with any surgical procedure, if infection does occur it may be treated with antibiotics.

Hair Loss
Hair loss is uncommon and may occur in and around the scar and temple areas.  No hair is cut during the procedure.  Any hair loss resulting is usually temporary. 

Delayed wound healing
Rarely, a small part of the scar behind the ear may take a little longer to heal.  This is sometimes the case in smokers.

You will be admitted to hospital on the morning of surgery and Mr Sojitra along with his consultant anaesthetist will come and discuss the surgical plan again with you.  You will usually be required to have a general anaesthetic.  The operation will take approximately 1 hour.  Your surgeon will inject local anaesthetic and adrenaline around your ear once you are asleep.  This helps reduce bleeding during your operation as well as reducing pain and improving your post operative comfort.  Cosmetic Surgeons London always puts your comfort first.  The incision is made behind the ear and the scar will lie in the crease behind your ear.  The ear is re-shaped by moulding the cartilage using permanent stitches which remain under the skin.  All the stitches used on the skin behind the ear will dissolve to maximise comfort for you.  You will wake up with a head bandage.  You will need to have a friend or family member to drive you to and from the hospital and to look after you for the first 24-48 hours following surgery. 

Recovery
You will experience some degree of bruising, swelling, and discomfort around your ears.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  It is important not to get the dressings wet after surgery and you will be advised on showering.  It is important to realise that should one ear become more uncomfortable than the other under the bandages then you must return to hospital to ensure that there is not an underlying blood collection (which can happen on rare occasions).  Depending on your type of work, you may return to work between 7-14 days after your procedure.  No heavy lifting, straining or strenuous exercise for 6 weeks.  Your surgeon will see you 7days after surgery at which point the head bandage will be removed and you will be able to first look at your new set of ears.  Your surgeon will closely follow your recovery.  You must wear a head band (like a tennis band) for 6 weeks at night.

Safety and potential complications?

Scars
Scarring on the whole is not problematic.  A scar will always exist and will be hidden in the crease behind the ear.  Certain individuals e.g. afro-carribean races and red haired individuals are at a higher risk of forming excess scar tissue (keloid or hypertrophic scars).  Should these scars develop excellent advice and treatment will be provided by Mr Sojitra. 

Bleeding
Rarely, small blood vessels can open up after surgery.  This may come to light in the first 24-48 hours by a feeling that one of the ears is more uncomfortable than the other.  If this is the case you must come to the hospital and be assessed.  If it is found that you have a collection of blood (haematoma) then this is washed out in the operating theatre.  No permanent side effects will occur and your otoplasty result will be unaffected.

Infection
This is an uncommon problem in ear correction surgery.  Antibiotics rarely need to be given. 

Alteration in skin sensation over the ear
The area over the ear where the skin has been lifted is often numb immediately after surgery.  This gradually recovers back to normal as the swelling subsides. 

Revisional Procedure
In approximately 5% patients a further procedure will be required to correct minor asymmetries.

Recovery
You will experience varying degrees of bruising, swelling, and some tenderness all of which improve by the day.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  You will be up and moving the day after surgery.  It is important not to get the dressings wet after surgery and you will be advised on showering.  Depending on your type of work, you may return to work between 14-21 days after your procedure.  No heavy lifting or strenuous exercise for 6 weeks.  It will take approximately 3 months before the swelling completely subsides and the final result seen.  The scar will continue to improve for up to 18 months and you are encouraged to firmly massage the scar after 4 weeks.  Your surgeon will see you 1 week after surgery at which point the dressings will be reduced and again at 2 weeks.  At 2 weeks the stitches, which are dissolvable, will be trimmed flush with the skin so they cannot be seen.  All dressings will be discarded at this stage.  Your surgeon will closely follow your recovery to ensure you are able to resume normal exercising and activities.  It is suggested you wear tight fitting garments for 6 weeks to minimise swelling.

Most patients experience some alteration in the sensation and “pins and needles” around the scar following surgery.  The area may become over or under sensitive.  In the majority of patients sensation becomes normal over time.

Deep Vein Thrombosis (DVT)

Recovery
You will experience varying degrees of bruising, swelling, and some tenderness all of which improve by the day.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  It is important not to get the dressings wet after surgery and you will be advised on showering.  Depending on your type of work, you may return to work between 2-14 days after your procedure depending on the amount and area of fat removed and injected.  No heavy lifting or strenuous exercise for 6 weeks.  The injected fat does not all take and survive and on average 60% will remain at 6 months.  Therefore you will require repeat ‘top-up’ procedures until the final permanent desired result has been attained.  Your surgeon will see you after 1 week.  All dressings will be discarded at this stage.  Your surgeon will closely follow your recovery.

Liposuction Body Contouring

What does liposuction involve?
Liposuction is usually performed using a general anaesthetic and depending on how much fat removal is performed you may go home the same day of after one night in hospital. 

How is fat removal performed?
Liposuction can be performed on its own or alongside other cosmetic procedures.  Fat is removed by first inserting a small, hollow tube (called a Cannula) through one or more tiny incisions.  Incisions are usually less than one centimetre in length and are placed as inconspicuously as possible.  The cannula is connected to a vacuum pressure unit (Liposuction) or syringe (Liposculpture).  The fat is therefore sucked out depending on the amount needed.

You will experience varying degrees of bruising, swelling, and some tenderness all of which improve by the day.  Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience.  You will be up and moving the day after surgery.  It is important not to get the dressings wet after surgery and you will be advised on showering.  Depending on your type of work, you may return to work between 14-21 days after your procedure.  No heavy lifting or strenuous exercise for 6 weeks.  It will take approximately 3 months before the swelling completely subsides and the final result seen.  The scar will continue to improve for up to 18 months and you are encouraged to firmly massage the scar after 4 weeks.  Your surgeon will see you 1 week after surgery at which point the dressings will be reduced and again at 2 weeks.  At 2 weeks the stitches, which are dissolvable, will be trimmed flush with the skin so they cannot be seen.  All dressings will be discarded at this stage.  Your surgeon will closely follow your recovery to ensure you are able to resume normal exercising and activities.  It is suggested you wear an abdominal support garment for 6 weeks. 

Safety and potential complications?

Scars
Scarring on the whole is not problematic.  A scar will always exist and will be hidden in the “bikini-line”.  Certain individuals e.g. afro-carribean races and red haired individuals are at a higher risk of forming excess scar tissue (keloid or hypertrophic scars).  Should these scars develop excellent advice and treatment will be provided by Mr Sojitra. 

Infection
This is an uncommon event and occurs in perhaps less than 1% cases.  Antibiotics are given during induction of the general anaesthetic.

Bleeding
Rarely, small blood vessels that have been cauterised during surgery open up afterward.  This results in a collection of blood (haematoma) that needs to be removed.  This involves a return to the operating theatre where the blood collection is washed out.  There are no permanent side effects due to this occurrence should it occur. 

Delayed Wound Healing
This can occur in the lower central part of the scar and is more common in smokers as smoking impairs wound healing.  When this happens, dressings are often applied by the nurse and the wound often heals without any surgical intervention.

Deep Vein Thrombosis (DVT)
Any general anaesthetic procedure is associated with a small risk of DVT which can potentially move to the lungs (Pulmonary Embolus).  Here at Cosmetic Surgeons London we attempt to minimise this risk by the use of compression stockings before and after surgery, use of pneumatic compression devices during surgery and motivating the patient to mobilise as soon as possible after surgery.
