Your nose is a very prominent part of youe face and many of you may not like the appearance of it. It may be the size or shape you may not like. You may have a hump on your nose or your nostrils may be too wide. Disproportion in any of its parts will result in deformity of some degree. How is nose reshaping or rhinoplasty performed?
Rhinoplasty can be performed by "closed" (where the scars lies inside your nose and cannot be seen) or "open" means (where the scars lie inside your nose and a small scar on your columella which is the skin area underneath your nose). This small scar on your columella heals very well and is almost invisible.
You (the Patient's) Journey
Before the operation
You will have seen Mr Sojitra on more than one occasion to fully answer all your questions. It is important to avoid medications, if possible, such as aspirin and non-steroidal anti-inflammatories (NSAIDs) e.g Ibuprofen (Nurofen), Sodium Diclofenac (Voltarol) for 1 week before surgery. These tablets may increase bleeding during and after surgery. It is advisable to stop smoking 2 weeks before and after surgery from both a wound healing and an anaesthetic perspective.
The operation
You will be admitted to hospital on the morning of surgery and Mr Sojitra along with his consultant anaesthetist will come and discuss the surgical plan again with you and make some marks on your skin with pen. You will be required to have a general anaesthetic. The operation will take approximately 1 - 3 hours. Your surgeon will mark on your nose where the incision lines will be and inject local anaesthetic and adrenaline. This helps reduce bleeding during your operation as well as reducing pain and improving your post operative comfort. Cosmetic Surgeons London always puts your comfort first. All the stitches used inside your nose will be dissolving in order to maximise comfort for you. Some fine stitches will be used that will need to be removed in 5 days. You will wake up with a plastic moulded splint taped to your nose to help reduce swelling and keep the shape of your new nose. It is best you take it easy and stay in hospital overnight. You will need to have a friend or family member to drive you to and from the hospital and to look after you for the first 24-48 hours following surgery.
Recovery
You will experience some degree of bruising, swelling, and discomfort around your eyes and cheek all of which improve by the day. The bruising should subside after 1 week. You may put make-up on after a few days if you wish. Our consultant anaesthetist will prescribe you adequate medication to take home to alleviate any discomfort you may experience. You are encouraged to rest the first night in hospital. It is important not to get the dressings wet after surgery and you will be advised on showering. Depending on your type of work, you may return to work between 7-14 days after your procedure. No heavy lifting, straining or strenuous exercise for 6 weeks. It can take several months before the swelling completely subsides and the final result seen. The stitches will be removed around 5 days. Your surgeon will see you 7-10 days after surgery at which point the plastic splint will be removed and you will be able to first look at your new nose. Your surgeon will closely follow your recovery.
Safety and potential complications?
Scars
Scarring on the whole is not problematic.
Bleeding
Rarely, small blood vessels can open up after surgery. This may come to light as a small nose bleed and often settles itself.
Infection
This is an uncommon problem in nose reshaping or rhinoplasty surgery. Antibiotics rarely need to be given.
Alteration in sense of smell
Your sense of smell can be altered in the early post operative period. Normally, this returns to normal reasonably quickly.
Alteration in skin sensation over tip of nose
The area over the tip of the nose where the skin has been lifted is often numb immediately after surgery. This gradually recovers back to normal as the swelling subsides.
Revisional Procedure
In approximately 5% patients a further procedure will be required to correct minor irregularities.
